A Long Term Relationship

Introductory Forms

We would like you to fill out introductory forms before arriving at our office.  You can print them here, click here, or you can call our office, 215-368-4434, to have them mailed to you.

Your First Visit

All new patients will meet with one of the providers for their first visit and a discussion of your health history will begin or we will take care of your acute/urgent care need.   We will likely complete your exam and discussion of blood and other work at the next office visit.

Completion of forms in advance of your appointment will allow for more time with the doctor.  Please bring with you your I.D. and insurance information.  Co-pays are required by insurances and are due at the time of the office visit.  Patients that arrive more than 15 minutes late for physicals may be asked to rescheduled.

Failure to keep an appointment without calling will be charged a $20.00 "no-show" fee as our appointment spaces are limited. If a patient fails to show up for three appointments they may be asked to seek a new family practice provider.

Annual Physical Exams

Some but not all patients are expected to have an annual physical exam and any appropriate screening tests on a regular basis depending on your health history and the current guidelines. These will be discussed with you.

Annual health maintenance and laboratory tests are recommended and sometimes required by commercial insurances and Medicare, almost always at no cost to you except for a deductible for certain items.
